The management of uncomplicated choledocholithiasis in elderly patients remains controversial due to uncertain clinical benefits of intervention and higher complication rates associated with ERCP. This study emulates a target trial to compare early ERCP with observation in this patient population. The findings suggest that observation may lead to better outcomes in a significant proportion of cases, raising questions about the routine use of ERCP in elderly patients with this condition.
