Uganda Police
This form, designated as Police Form 3 by the Uganda Police, serves as a standardized tool for documenting medical examinations of individuals involved in police cases. It is divided into two parts: Part (a) is for the police officer to request the examination, specifying the victim and case details. Part (b) is to be completed by the medical/health practitioner, including victim particulars, history, physical examination findings, injury classification (Harm, Grievous Harm, Dangerous Harm, Maim), and any other relevant observations. The form also includes a pictogram for injury documentation and definitions of injury classifications.
