Beta-blockers are widely used in managing both supraventricular and ventricular arrhythmias, yet they are not a homogeneous class of drugs. This review highlights the differences in beta1-receptor selectivity, lipophilicity, half-life, and other properties among beta-blockers, and how these differences impact their efficacy in treating specific arrhythmias. The evidence is drawn from a comprehensive search of multiple databases, prioritizing randomized controlled trials and contemporary guidelines, and is synthesized qualitatively by arrhythmia type.
